VNA Homecare Options is a managed long-term care (MLTC) Medicaid plan that provides care for elderly people who wish to remain in their homes. We cover a broad scope of services for the elderly, including home care as well as community-based care. We work with you to design an individualized plan of care and services to keep you at home safely. And, we will work with you and your physician to coordinate your in-home care.
We know that staying at home is an important part of maintaining a feeling of independence for the elderly. As people grow older, it becomes more difficult for them to care for themselves and to remain in their own homes safely. That’s why we strive to provide the best elderly care available for you or your loved one, right in your own home. We will guide you through the first steps of elderly care, and help you determine the specific types of home care services that will best benefit you. We’ll design a care plan specially designed for the elderly patient, that will meet all of your needs and help you stay safely at home.
Managed long-term care plans offer a wide array of services, including both medical and non-medical elderly care. These services are paid for through Medicaid. In order to qualify for a MLTC, elderly patients must be eligible for nursing home care.

You are eligible for managed long-term care (MLTC) from VNA Homecare Options if you:
- Are age 18 or older;
- Are eligible for nursing home level of care (In some cases may be required);
- Can stay safely at home and in the community at the time you join the plan;
- Are expected to need long-term care services for at least 120 days;
- Are Medicaid-eligible;
- Live in the area served by the plan
